How to Create and Use Custom Questions in a 360 Feedback Review

Testlify now allows you to create fully customized 360 Feedback questionnaires using your own company tests and questions. Once created, these questionnaires can be reused across any assessment that utilizes the 360 Feedback Review feature.

What is a 360 Feedback Questionnaire?

A 360 Feedback questionnaire allows managers, peers, stakeholders, or reviewers to provide structured feedback on candidates using predefined questions and rating criteria.

You can now:

  • Create your own custom 360 feedback questionnaires
  • Add company-specific questions
  • Reuse questionnaires across multiple assessments
  • Maintain consistency in feedback collection

Step 1: Create a New Company Test

  1. Navigate to Library.
  2. Click Create company test.

Step 2: Enable 360 Feedback Mode

On the Basic tab:

  1. Enter a Test Name.
  2. Select a suitable Test Type.
  3. Enable Use as 360 feedback review of candidates.

Once enabled, the test becomes a dedicated 360 Feedback questionnaire.

Important

Only tests with this option enabled will appear in the 360 Feedback question library and assessment setup.

Step 3: Add Custom Questions

Click Next to open the Questions tab.

The following question types are currently supported:

  • Single Select
  • Multiple Select
  • Yes / No
  • Rating
  • Dropdown

You can:

  • Create brand-new custom questions
  • Build role-specific feedback forms
  • Standardize review criteria across teams

Select the candidate's proficiency level.

Step 4: Configure Settings

After adding your questions:

  1. Click Next.
  2. Configure any desired test settings.
  3. Click Save.

Important

The questionnaire must be saved before it becomes available for use in assessments.

Step 5: Use the Questionnaire in Any Assessment

Once saved, your 360 Feedback questionnaire can be used in any assessment where the 360 Feedback Review feature is enabled.

When selecting a questionnaire:

  • Only tests marked as Use as 360 feedback review of candidates will appear.
  • Standard tests will not be shown.

This helps ensure only relevant 360 Feedback questionnaires are available during setup.

Question Library Behavior

The 360 Feedback question library will only display:

  • Company tests configured as 360 Feedback questionnaires
  • Questions created specifically for 360 Feedback reviews

Regular assessment tests and unrelated library content are excluded.

Test Library Search & Filters

The Testlify team has also improved the test library filtering experience.

What's Fixed?

When searching for:

  • "360"
  • "360 feedback"
  • Other related keywords

the appropriate 360 Feedback questionnaires will now appear correctly in search results and library filters.

Best Practices

  • Create separate questionnaires for different roles or departments.
  • Use Rating questions for measurable feedback.
  • Keep questionnaires concise to improve response rates.
  • Reuse saved questionnaires across multiple assessments for consistency.
