Add consent checkboxes with linked terms and documents
Ask candidates to agree to your terms, privacy notice, or any other policy before they start an assessment. You can turn part of a checkbox label into a link that points to a web page or an uploaded document, so candidates can read the full text before they agree.
What you can do
- Add one or more Checkbox fields to the Information tab of an assessment, for example one for your privacy notice and another for your terms and conditions.
- Link any part of the checkbox label to a URL or to an uploaded file (PDF, DOCX, or PNG, up to 20 MB).

Add a checkbox with a linked label

Open your assessment, go to Setup assessment → Settings, and select the Information tab.
- Click Add at the top right of Additional information. The Add field window opens.

Set Type to Checkbox. Type is the first field in the window. The link options only appear for Checkbox fields.

In Field label, type the text candidates will see next to the checkbox. For example: I agree to Testlify's terms and conditions
- Select the words you want to turn into a link (for example, terms and conditions). The Link selected text option, at the top right of the Field label box, becomes active once text is selected.

Click Link selected text. The words are styled as a link, Unlink replaces Link selected text, and the Attach link or document field appears below the label.
- In Attach link or document for "[your selected text]", use the dropdown on the left to choose one option:
- Link: paste the URL of the page you want to show.
- File: click Upload file and choose one file. Accepted formats are PDF, DOCX, and PNG, up to 20 MB.

- (Optional) Turn on Mark as required, Hide from candidates, or Hide from reports.
- Click Save.

Things to know when setting up the link
| Situation | What happens |
| No text is linked in the label | The "Attach link or document" field stays hidden. |
| You switch between Link and File | The value you entered in the other option is cleared. |
| You upload a file | The file name is shown with a ✕ so you can remove it. Only one file can be attached per link. |
| You want to remove the link | Click "Unlink" at the top right of the Field label box. The text goes back to plain text and the attach field is hidden again. |
Save-time checks
Testlify checks that your label and its destination match before saving:
- If part of the label is linked but no URL or file is attached, you will see: "Add a link or document for your linked text before saving."
- If you have entered a URL or uploaded a file but no text in the label is linked, you will see: "Select and link some label text before saving."
Fix the issue and click Save again.
What candidates see
- Your checkbox appears on the form candidates complete before they start the assessment, alongside fields such as first name, last name, and email. The linked words are shown as a hyperlink.

- When candidates click the linked text:
- If it is a web link, the page opens in a new browser tab.
- If it is an uploaded file, a window opens over the form. The linked text is the window title and the document is displayed inside it.

- Candidates click the ✕ at the top right of the window to close it and return to the form.
- If you marked the checkbox as required, candidates cannot continue until they tick it.
| Why do web links open in a new tab? Many websites (for example, YouTube and most company sites) block being displayed inside another page for security reasons. To make sure every link works, Testlify always opens URLs in a new tab. Uploaded PDF and DOCX files are shown directly in the window. |
Best practices for consent checkboxes:
- Add one checkbox per agreement. Separate checkboxes for your privacy notice, terms, and any supplemental notices make it clear what each candidate has agreed to.

Frequently asked questions:
Can I link more than one part of the same label?
Each checkbox has a single "Attach link or document" field, so it holds one link or file. If you need to point to several documents, add a separate checkbox for each one.
Can I attach more than one file to a single link?
No. Each link supports either one URL or one file.
Which file types can I upload?
PDF, DOCX, and PNG, up to 20 MB.
Why can't I see the "Attach link or document" field?
It only appears after you select text in the label and click "Link selected text". If you click "Unlink", the field is hidden again.
Does this work for all field types?
No. Linking label text is available for Checkbox fields only. The options "Mark as required", "Hide from candidates", and "Hide from reports" work for all field types.
Can candidates open the linked document without ticking the checkbox?
Yes. Clicking the link never changes the checkbox. Candidates tick it themselves once they are ready to agree.
